Beacon Health Provides Testimony on Capitol Hill


At the request of Senator Susan Collins (RME) a leader from Beacon Health, a member of EMHS, is testifying before the United States Senate Committee on Aging, in Washington DC.

Beacon Health’s Pioneer accountable care organization is one of seven Pioneer ACOs in the country to participate in the Medicare skilled nursing facility three day waiver program. The waiver allows primary care providers to support the right level of care for their Pioneer Medicare patients, without meeting the CMS requirement for a three night stay in a hospital. The waiver allows a provider to move a patient into a skilled nursing facility from their primary care practice, a hospital emergency department, or following a night or two in a hospital.

Tori Gaetani, RN, Beacon Health's director of care coordination, is sharing with Senators Beacon Health’s’ success with the waiver and how it is enhancing care delivery. The waiver program has helped Beacon deepen relationships between primary care teams, patients and families, and other providers in the community who help support our nearly 29,000 Medicare patients who today are living their healthiest lives possible.

The nearly 200 Beacon Health patients, who have benefited from this waiver, have said it was just what they needed to continue living independently at home. The cost savings to Medicare for just Beacon Health patients alone is well into the in the tens of thousands of dollars.

The Beacon Health Pioneer network includes 12 hospitals, more than 300 primary care providers from 72 practice locations, working together across 67 Maine cities and towns to coordinate care, improve quality and patient experiences, and manage costs.

Beacon Health Pioneer patients have access to nurse care coordinators, transitions of care coordinators, health coaches, and social workers, all of whom are part of a patient’s healthcare team. This helps ensure patients are empowered with not only high-quality care, but also the information and tools they need to be engaged in their own health.
