A Proven Model to Better Health Care Coordination Recognized as “Best Program”


BREWER, Maine – June 3, 2014 – It is with great pride that we announce our Beacon Health Case Management program has received accreditation from NCQA (National Committee for Quality Assurance). Since 1990, NCQA has been considered the gold standard for improving healthcare across the country. Beacon Health is the first care delivery system to achieve this distinction.

“We believe in our care model and our nurses achieving this recognition is proof that we know what we are doing and it’s working. The best part is that our patients are feeling the difference,” shares Tori Gaetani, RN, Beacon Health director of nurse care coordination. 

“Case management accreditation moves us closer to measuring quality across population health management initiatives,” said Margaret E. O’Kane, president, NCQA. “Not only does it add value to existing quality improvement efforts; it also demonstrates an organization’s commitment to the highest degree of improving the quality of their patients’ care.”

Beacon Health nurse care coordinators are embedded throughout Maine in our nearly than 40 primary care practices. They are an integral part of our healthcare transformation. Since this added support has been a benefit to the more than 10,000 EMHS employees, we have recorded a decrease in hospital admissions by 27 percent for this population. Additionally, emergency department visits have decreased by nearly eight percent among this same population.
